IC4 is an interdisciplinary team of doctors, nurses, social workers and patient and family representatives working together with Indiana Medicaid, Title V Children with Special Health Care Services and the Bureau of Disabilities Services and other community partners.
Mission
To improve the quality of life and care for Indiana patients insured by Medicaid who have high needs, by coaching and embedding nurse care coordinators within primary care practices statewide and across health systems.

Our work will improve the opportunity for children with medical complexity and adults with intellectual disabilities to live their best lives.
