The Pediatric Palliative Care Team is an interdisciplinary consult-based medical team that includes physicians, advanced-practice nurses, and social workers. This team is dedicated to the mission of helping patients achieve quality of life in the face of life-threatening and/or life-limiting medical conditions and provides assistance in helping patients/families communicate their goals of care with their treatment teams, assists in complicated symptom management, provides an extra layer of support during times of difficulty, and offers guidance in advance care planning and end-of-life/hospice care when appropriate.
