Phase 2 (Y3) students in Indianapolis may be placed at regional campuses for multiple clerkships, up to 3 months, not including Family Medicine.
If you have extenuating circumstances (e.g. dependent care or medical reasons) that may qualify for a travel exemption. You must complete this form by 5 PM EST on Tuesday, October 22nd.
Please note that you can edit your submission until the deadline, and the last one will be reviewed. Please consider discussing this with your Lead Advisor. Requests will be reviewed, and decisions will be made in early November.
