HEAL-R implements innovative strategies in health care delivery that directly address social and structural inequities. Our focus lies in developing novel approaches that identify and reduce disparities in patient access, experience and outcomes of care. By collaborating with IU Health’s Office of Health Equity Research and Engagement (HERE) and community organizations, we aim to implement sustainable and scalable solutions that create a more equitable healthcare landscape, ensuring that every patient receives the care they need, regardless of their background or circumstances.
Indianapolis Health Equity Access outReach & Treatment (iHEART) Collaborative
To make Indiana one of the healthiest states in the nation, the IU Health Office of Health Equity Research and Engagement (HERE) alongside the IU Health Community Health Team, Cardiovascular Institute and Congregational Care Network has focused efforts on eliminating disparities in hypertension, a key determinant of overall cardiovascular health. The iHEART Collaborative is a cardiovascular health initiative focused on reducing cardiovascular disease (CVD) rates in three Indianapolis communities. Communities of color, particularly African American communities, face elevated hypertension rates and barriers to accessing care. A coordinated focus on mitigating CVD disparities in communities facing the most acute disease burden will directly impact our progress toward improving Hoosier health.
