CHOICE Program

Changing Health Outcomes through Integrated Care Excellence (CHOICE) provides the following services to primary care clinics:
  • Brief psychotherapy conducted by master's level therapists and psychologists
  • Proactive medication recommendations from psychiatrists to primary care providers
  • Diagnostic clarification
  • Triage and referral to appropriate community resources if applicable

The Collaborative Care Model

Collaborative Care is a model of care designed to augment mental health treatment in primary care clinics. The model is based on the evidence-based AIMS model that has been shown to increase access to care and improve mental health outcomes.

Accessing CHOICE Services

Patients with a primary care provider in the following clinics can access CHOICE services:

  • Riley Outpatient Clinic
  • Methodist Family Medicine Center
  • Georgetown Medical Center
  • Capital Center/Artistry
