If a client revokes or withdraws interest in CME accreditation of an approved activity less than 30 days of the scheduled activity date, a cancellation fee may be charged to the client. The cancellation fee amount is based on 50 percent of the CME base accreditation fee quoted on the original cost estimate. Cancellations should be made via email or phone to the assigned Activity Coordinator.
Materials and resources are available to assist CME activity planners in the planning process. The IUSM CME Application and Planning Worksheet is the initial form required to start planning a CME activity. It is designed to help facilitate analyses and guide the planning process. Other forms are required depending on the type of activity, the educational format selected, and whether or not commercial support is used.
All activities require the following forms.
For each presenter, author or educational partner who is in a position to control content, a Disclosure of Relevant Financial Relationships must be submitted.
Disclosure information provided to learners prior to the start of the educational activity.
A standard evaluation questionnaire used to illicit basic information following an activity. This is the minimum required. Methods of evaluation are determined by the objectives and desired results.
