1st Step - IUPUI Application

Please make sure you have read this step before completing the HPP Application! Applicants must submit an application for enrollment to the IUPUI campus  http://enroll.iupui.edu/admissions/apply. Actively enrolled students on the IUPUI campus do not need to complete this process.  However, if you last enrolled at IUPUI more than one year ago, you must update your enrollment status.  If you need further assistance with your university application, please contact their office at (317) 274-4591 or enroll@iupui.edu. When completing the IUPUI application please pay close attention to the semester entering IUPUI to enroll in classes. This should match the program's term of entry listed on the applicaton unless you need to enroll in some additional course requirements before starting the program.

2nd Step - Health Professions Programs Application & Required Forms

The IU School of Medicine Health Professions Programs application packet for entry in spring, summer, or fall 2010 is available for download in Adobe Acrobat format only.  To ensure optimum Adobe Acrobat capabilities please download the latest free Adobe Reader. You may choose to fill-in the form online or print out and fill-in using a ball point pen. The packet is eight pages total;two pages of instructions and six pages to be completed by the applicant. Please make sure that you submit a completed application packet by the specific program's application deadline. APPLICATION

Radiography, AS:Pre-Interview Questionnaire Paramedic Science, AS:  Must submit a copy of your State or National EMT Certification & your state-issued driver's license. Nuclear Medicine Technology, BS:Pre-Interview Questionnaire Radiation Therapy, BS:Radiation Oncology Clinical Observation Form (Applicants must submit two separate forms representing observations at two different locations). Respiratory Therapy, BS:Respiratory Therapy Clinical Observation Form                                           List of Clinical Observation Sites NOTE: The B.S. in Clinical Laboratory Science, B.S. in Cytotechnology, and B.S. in Medical Imaging Technology do not require the submission of any additional materials.
